Objective Evidence

Evidence needs to be provided to verify that Preventive Corrective Actions were put into place and that they are effective in preventing recurrence.

NIP 7-03 defines the specific requirements for submittal, however, general guidance is:

INCLUDE OBJECTIVE EVIDENCE OF ALL ACTIONS DESCRIBED!

For example:

- If procedures were revised, attach only the pages that were changed and the approval page. Do not send the entire procedure, just the relevant parts.

- If personnel were trained, send us a syllabus/agenda/outline and the record of who was trained.

- If you added labels to tanks, photos may be appropriate.

Submittal of objective evidence allows a more thorough review. If you feel attaching documentation will assist the Staff Engineer and Task Group close your finding, send the information.
